The practice accepts applications from patients living in the Condorrat area of Cumbernauld.
We may also accept patients from Balloch, Blackwood, Carrickstone, Craigmarloch, Dullatur, Eastfield, Smithstone, The Village, Westerwood and Westfield.
In order to register with the practice you will need to download, print and complete a Registration Form, a New Patient Questionnaire (over 16s only) and a Patient Services Registration Form.
These can also be collected from reception.
Please return the completed forms with proof of ID to reception.
Once we have received your completed form your application will be reviewed and we will contact you within 14 days to move onto the next step of the process.
Please note that until your registration is complete you should continue to see your current GP and request medication from them.
If you move out of the practice area you will need to register with a new practice as soon as possible. You can do this by approaching a surgery closer to your new address and asking to register or if you are having difficulties in being accepted please write to Practitioner Services at the following address:
