Prescriptions can be ordered in one of three ways:
Online – register for a Patient Services account by completing an application form and handing it in at reception.
Please note this is not the same as emailing your prescription request. The email service was withdrawn on 1st December 2019. All patients currently using email have been receiving automated responses since September advising them of this and encouraging them to register for Patient Services.
By posting your re-order slip in the post box to the right of reception desk or complete one of the slips available.
When a prescription for repeat items is issued the re-order slip is on the right-hand side. Please detach this before handing your prescription into the pharmacy and keep for future use. If completing one of the slips please ensure you write your name, date of birth and the name of the medication you are requesting. This may seem obvious but we regularly get slips with nothing more than a name on it!
Make arrangements with a pharmacy of your choice to order and collect prescriptions on your behalf.
If you wish your prescription to be collected by one of the local pharmacies on your behalf then please make this clear to us when ordering. Your preferences will be noted in your medical record and any prescription issued thereafter will have the pharmacy’s name on it.
If you have nominated a pharmacy to collect your prescription please do not come to the surgery to collect it as it will have been forwarded as you asked.
